Tristan has never hurt anyone. Unless you count the self harm he has done to himself. If you walked in on someone you liked fucking someone else; chances are you would be at least a little upset at first. It’s a natural human response.
The last girl Tristan was in a relationship with was Siora. She started sneaking around behind his back and fucking other guys. It really upset him and he had a very hard time coping with it. He cut his arms to pieces and of course that is not a healthy response. Possibly walking in on San fucking someone else temporarily reopened those old mental wounds…
