Nope again, although I have not been closely inspecting Kostja cock like it appears you have been. 
The lesions do not have to be easily visible but you are correct they do have to be viable. The lesions can be small enough to not easily be detected by the human eye. They could also be located in a personās mouth. Due to the picture quality of the cams and/or where the lesions could be located ie. their mouth. The lesions could easily go undetected by us the viewers.
